    Essential Functions:

    • Assess nature of clients' situations by interviewing and reviewing personal history
    • Develop and execute individual care plans
    • Determine appropriate milestones to gauge client progress
    • Communicate client updates to all relevant parties
    • Facilitate referrals to other healthcare professionals and programs
    • Maintain accurate client documentation
    • Serves as a brand ambassador for Vitra reflecting Vitra’s vision, mission, and values.
    • Shows a genuine interest and compassion for the communities we serve and commitment to the diversity of our clients and team members.

    Education, Experience and Licensure Requirements:

    • LICSW, LCSW, or LSW
    • Previous experience in social work, counseling, or other related fields
    • Compassionate and caring demeanor
    • Ability to build rapport with clients
    • Strong leadership qualities
    • Excellent written and verbal communication skills
    • Sensitivity and understanding regarding the needs of elder and disabled persons and families.
    • Ability to work well with persons of diverse backgrounds.
    • Ability to exercise sound judgment under pressure.
    • Exceptional organizational and communication skills, basic computer, and management skills
    • Familiarity with the Department of Public Health, MassHealth and Child and Adult Care Food Program (CACFP) regulations and ability to put them into practice.
    • Familiarity with provider services such as Senior Care Options (SCO), One Care, Area Service Access Points (ASAP) regulatory compliance and audit readiness
    • Demonstrated robust experience in Massachusetts regulations.
    • Excellent customer service, interpersonal, and communication skills.
    • Bilingual in English and secondary language preferred
