Essential Functions:
Our GAFC Caregiver position provides care under the supervision of a Registered Nurse (RN)
GAFC Caregivers assist with Instrumental Activities of Daily Living (IADLs) and Activities of Daily Living (ADLs) including skilled tasks when applicable
Instrumental Activities of Daily Living (IADLs) are environment focused and may include: meal preparation, light housekeeping, laundry, running errands, grocery shopping, etc
Activities of Daily Living are personal care-focused and may include: assisting clients with bathing, grooming, dressing, ambulation, eating, and toileting
Offer companionship to the clients
Provide medication reminders
Keeping notes and reporting any changes in health to your supervisor
Utilize the electronic visit verification system, Skedulo, mobile app to view and accept client assignments and complete the assignment including but not limited to: utilizing geo-track to track travel time and mileage, executing all forms/tasks/etc., and obtaining required signatures
Complete all required in-person training and competency evaluations
Any other duties as assigned
Region Servicing: Region 5
Qualifications:
Must be available to work a set shift schedule (e.g., 8:00 am - 4:00 pm) with rotating weekend coverage
Must be able to lift and carry up to 50 pounds
Must be able to travel to clients within assigned region (minimum 20-miles radius)
Must have a valid driver's license and access to a reliable vehicle
Must maintain a working mobile device with cellular data access and downloaded Skedulo app with geo-tracking features enabled
Are current with CPR certification (this can be provided)
Have proven ability to behave with respect and compassion
Possess a professional and upbeat attitude
Personal Care Attendant - PCA, Homemaker, Home Health Aide - HHA, or Certified Nursing Assistant - CNA experience a plus!
